Wooster Community Hospital would like to SHINE THE SPOTLIGHT on the members of our integrated care coordination staff.
Our RN case managers and licensed social workers work as a collaborative team to help patients and their families navigate an often unfamiliar and confusing medical journey. They provide support for patients and their families who are experiencing psychological, social, financial, or physical challenges that arise as a result of medical conditions and/or hospitalization.
Each member of the team helps navigate the challenges that can keep you and your loved ones from the health care you need.
Below are just some of the amazing RN case managers and licensed social workers we’d like to give a shout-out to! Thank you for everything you do for our community!
